Please note that pre-authorizations are not a guarantee of payment and only your insurance carrier is able to make such determinations.

Based on the daily activities and needs for our residents, recommended visiting hours are 8:00 a.m. until 8:00 p.m. This provides our residents with the time they need to prepare for their day and obtain the rest they need to participate in their treatment and daily activities. Special accommodations can be arranged given special circumstance.

Assisting friends and family with the decision is an essential element of post-acute care. Our Admissions team accepts admissions seven days a week.

The Pavilion at Creekwood is a skilled nursing facility located in Mansfield, Texas. Our professional staff and independent physicians and consultants at The Pavilion at Creekwood are committed to our residents and their families. Our dedicated Short-Term Rehabilitation Program at The Pavilion at Creekwood consists of an interdisciplinary team that evaluates each resident's unique situation and works to create a cohesive and individualized plan for success. During treatments, our therapists will work to improve independence, strength, and confidence so our patients can transition to the least restrictive environment upon discharge with the goal of returning to their prior living environment.

Our Long-Term Care Program was designed to assist those who may benefit from 24-hour nursing, meal assistance, medication management, as well as other activities required for daily living. We strive to maintain a very comfortable living environment for our residents with an emphasis on health and wellness.
