Visitation

Based on the daily activities and needs for our residents, recommended visiting hours are 8:00 a.m. until 8:00 p.m.

  • Outdoor visitation is always an option, but please consider the weather forecasts.
  • Contact our Receptionist at (817) 779-6500 to schedule a compassionate care visit.

This provides our residents with the time they need to prepare for their day and obtain the rest they need to participate in their treatment and daily activities. Special accommodations can be arranged given special circumstance.

Assisting friends and family with the decision is an essential element of post-acute care. Our Admissions team accepts admissions seven days a week.

How often does a patient get rehabilitation?

Rehabilitation therapies are provided as prescribed by the resident’s physician depending on the resident’s needs. But a resident could receive therapy (physical, speech, occupational) up to six days a week depending on his/her needs.

What are visiting hours?

Based on the daily activities and needs for our residents, recommended visiting hours are 8:00 am until 8:00 pm. Other times may be accommodated and should be arranged in advance. We ask that visitors consider other residents when planning their visits.

Will I be able to use my own physician?

All of the independent physicians that provide care for residents at our facility must be credentialed. If your physician is not credentialed to follow residents at the facility, we are happy to work with him/her through that process. We also have a list of physicians that are credentialed who are available to serve you.

When can a resident be admitted?

Planned resident admissions are accepted 7 days a week.

What should I bring with me?

We encourage our residents and their families to bring items that will make their stay with us comfortable. Along with this, residents should bring any personal care items such as eye glasses, hearing aids, toiletries, enough clothing for 5-7 days of wear and comfortable shoes.
Additionally, we encourage all of our long term residents to personalize their room with pictures and decorations. Some items, such as throw rugs, electrical items, and bulky furniture may not be allowed, but we will gladly consult with you regarding items that can be included, as space and safety parameters permits.

What type of rooms do you offer?

We offer private suites for all skilled nursing (rehabilitation) patients. We offer semi-private rooms for our long-term residents.

Are pets allowed to visit?

We love pets at The Pavilion at Creekwood. Short visits are great for everyone. Any pets permitted in the building must have a current vaccination record and remain on a leash or in a proper carrier and in their owner’s control at all times.

Do you provide laundry services?

Yes, our daily rate includes laundry services, however the family is welcome to take care of the personal clothing if desired.

Can a family member stay overnight?

Based on the needs and schedules of our residents, overnight guests are not recommended. If a resident’s condition warrants an overnight stay, alternate accommodations may be made through the Administrator.
