Success Story – Nov – Ms. Shin

Ms. Shin came to The Pavilion at Creekwood after a hospitalization for a fall at home resulting in multiple fractures in her back, ribs, right forearm, and right hip which she underwent surgery for. Ms. Shin worked diligently with her therapists during her stay. She went from requiring a significant amount of assistance with all self-care skills and mobility to walking with a quad cane and completing her self-care skills independently prior to returning home.

Success Story – Sept – Mr. Nelson

Mr. Robert Nelson arrived at The Pavilion at Creekwood after a hospitalization for a fall that resulted in a significant spinal surgery. Mr. Nelson worked diligently with his therapists for two months and through his hard work and their support, he went from requiring a Hoyer lift to get out of bed and only tolerating 30 mins to supervision with a sliding board transfer and tolerating being out of bed for several hours.

Ms. Pierce arrived at The Pavilion at Creekwood after a hospital stay leaving her unable complete her daily tasks or walk without assistance. Ms. Pierce worked diligently with her therapists and through this hard work and their support, she has returned home able to walk with a walker and able to complete her daily tasks independently.

Ms. Kinder arrived at The Pavilion at Creekwood after an extensive, medically complex hospital stay leaving her unable to complete her daily tasks or walk without extensive assistance.
Ms. Kinder worked diligently with her therapists and through her hard work and their support, she has returned home walking with a walker and able to complete her daily tasks independently.
